Please Note: This is a part-time, independent contractor position designed for individuals seeking supplemental income. It is not a full-time, benefits-eligible position and is not intended to lead to full-time employment. This role offers valuable hands-on experience working with youth experiencing behavioral, emotional, and mental health challenges while supporting their Legal Authorized Representatives (LARs)

The Supported Living Support Worker plays a vital role in enhancing the quality of life for individuals with diverse needs.
This position involves providing direct support to youth and their family in their home by teaching social, communication, anger management, growth mindset, etc…skills to improve thinking and behavior and function in daily activities, ensuring their safety, comfort, and well-being.
The ideal candidate will be compassionate, patient, and dedicated to fostering independence while offering assistance to youth with a serious emotional disturbance (SED) in developing independence and integrating into the community by providing skills training in areas like personal hygiene, household chores, socialization, and communication, while also supporting the family caregiver in managing the youth’s disability-related needs, all within the framework of a Wraparound plan.
Responsibilities
- Provide curriculum based strength and skill based training to youth and family.
- Increase youth social skills to function successfully in their daily living activities, including personal care and hygiene.
- Monitor youth behavior and interaction and staff with the assigned facilitator during team meetings. Document and Report to facilitator if notice and observe concerning behaviors.
- Providing communication skills based training to enhance effective expression and understanding.
- Support youth and family in engaging with their community and participating in recreational activities.
- Facilitating social skills training to improve interaction with peers and community members.
- Supporting the development of problem-solving and coping mechanisms.
- Collaborating with family caregivers to identify and address specific needs related to the youth’s SED.
- Educating family members on strategies to support the youth’s development and behavior management.
- Working closely with the Wraparound team, including mainly the facilitator and other service providers on the youth team to ensure coordinated care.
- Monitoring progress towards goals outlined in the Wraparound plan and making adjustments as needed.
- Documenting all services provided and maintaining detailed progress notes.
- Assessing the youth’s needs and identifying areas where additional support may be required.
- Advocating for the youth’s needs to ensure access to appropriate services and community resources.
- Adhere to HIPAA regulations to maintain confidentiality of client information.
- Collaborate with families, assigned facilitator, and other support staff to develop personalized care plans.
Experience
- Previous experience as a CLS or in a youth facility is preferred but not mandatory.
- Experience providing direct support and skill and strength based learning for individuals with mental health, disabilities or special needs is an asset.
- A compassionate approach combined with strong communication skills is essential for success in this role.
This position require the provide to complete training before working with the youth and family, provide professional liability insurance, background check, CPR, Handle with Care or SAMA( Satori Alternatives to Managing Aggression ). All training is Free, but the Professional Liability Insurance and Background Check are the responsibility of the provider.

Services should be tailored to meet the unique needs of each youth based on their Wraparound plan.
Effective CLS requires close collaboration with the family, other service providers, and the Wraparound team.
