Your Choice At Home is an affiliate of Cobble Hill Life Center and a Certified Home Health Agency (CHHA). We’re culturally sensitive, and many of our staff come from within the community that they serve. Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services. As an Account Manager, you will promote maximized referrals through the design and implementation of outreach programs, including marketing collateral materials and measures successful execution of those initiatives.

Key Tasks Include

Strategically plans and executes strategies, outreach, and educational activities to maximize census

Participates in the development/review of promotional materials to support growth plans

Develops and maintains favorable relationships with key influencers in the community

Support the execution of the annual marketing plan, service line plan and budget

Work collaboratively with the Operations Manager and Director of Business Development, to plan and execute outreach programs in line with the CHHA’s growth initiatives

Manage numerous, multi-step projects simultaneously with the highest attention to detail

Possess a solid understanding of Your Choice at Home’s CHHA target population and insurance type

Serves as the designated representative for YCAH to the media and other outside interested parties and officials

Plans communications campaigns with primary referral sources

Implements educational programs, e.g., wellness and prevention, for primary target populations

Creates and disseminates educational newsletter for patients, families, and referral sources

Serve as liaison between service providers and patients, families, and referral sources

What We’re Looking For

Bachelor’s degree in marketing, Public Relations, Communication, or Public Health with focus on healthcare

2-3 years of Marketing or Public Relations experience

Previous experience in home care and/or hospital setting

Strong written and verbal communication skills

Must be willing to travel

Demonstrates the ability to multi-task and adhere to deadlines
